What is Clomid (Clomiphene Citrate)?
Clomid is a synthetic drug belonging to the category of selective estrogen receptor modulators (SERMs). Its active ingredient, clomiphene citrate, was developed in the early 1960s as a treatment for fertility disorders and is still widely used in the medical field today.

It is a non-steroidal compound that acts on the hypothalamus, interacting with estrogen receptors and modulating the hypothalamic-pituitary-gonadal (HPG) axis. From a chemical point of view, clomiphene citrate consists of two active isomers— zuclomifene and enclomifene —which contribute synergistically to its pharmacological action.

In addition to its traditional clinical use, clomiphene citrate has also become particularly well-known in the world of sports. In bodybuilding and strength sports, it is used off-label in post-cycle therapy (PCT) to restore endogenous testosterone production after cycles of anabolic steroids. It is important to note that the off-label use of clomiphene citrate carries risks and should be done under medical supervision.

Main effects and benefits of Clomid

Stimulation of gonadotropin release

Clomiphene citrate acts as a competitive antagonist of estrogen receptors in the hypothalamus, blocking the negative feedback of estrogen on GnRH secretion. This mechanism induces the anterior pituitary gland to release greater amounts of LH and FSH, hormones that are essential for male and female reproductive function.

Increase in endogenous testosterone

In men, it promotes a significant increase in serum testosterone levels by stimulating the Leydig cells in the testicles. This effect is particularly beneficial in cases of functional hypogonadism and during post-steroid recovery, helping to preserve strength, muscle mass, and overall well-being.

Ovulation induction

In women with anovulatory infertility, clomiphene citrate is one of the first-line treatments. Clinical studies show that it induces ovulation in 70–80% of cases, with pregnancy rates of 30–40%, especially in patients with polycystic ovary syndrome (PCOS)

Improvement of spermatogenesis

Elevated FSH levels promote spermatogenesis in men with infertility, improving sperm count, motility, and morphology

Clomid in Post-Cycle Therapy (PCT)

In bodybuilding, clomiphene citrate is used to counteract the suppression of the HPG axis caused by anabolic steroids. It helps normalize endogenous testosterone levels and reduces the risk of side effects associated with hypogonadism, such as fatigue, depression, and muscle loss.

Mechanism of action

  • Hypothalamic action: blockade of estrogen receptors → increased GnRH secretion

  • Pituitary action: increased release of LH and FSH
  • Gonadal action:

    • in women → follicular maturation and ovulation

    • in men → stimulation of testosterone and sperm production

This combined action makes clomiphene citrate effective in both men and women.

Main uses of Clomid

  • Post-cycle therapy (PCT)
  • Hypopituitarism and male hypogonadism

  • Male infertility (oligospermia)

  • Female anovulatory infertility (PCOS)

  • Secondary uses: male gynecomastia, support in certain oncology protocols

Dosage and administration

PCT in bodybuilding

  • 50 mg daily for 4–6 weeks
  • Often combined with tamoxifen to optimize recovery

Male hypopituitarism

  • 25–50 mg daily or every other day
  • Regular monitoring of hormone levels

Female infertility

  • 50 mg per day for 5 days, starting on the 5th day of the cycle
  • Maximum dose: 150 mg/day

Side effects

Generally rare and dose-dependent, they may include:

  • hot flashes
  • gastrointestinal disorders
  • Mood swings

  • Headache

  • temporary visual disturbances

Rare but serious events include ovarian hyperstimulation syndrome (OHSS) and thromboembolic events.
